A career highlight to lead this study on 10 year outcomes in almost 3500 men undergoing focal therapy, now published in @EUplatinum And fantastic to see the widespread UK media coverage today across TV, radio, and print! Many thanks to my collaborators and mentors 🙏
🚨 NEW IN @EUplatinum! 🚨 The largest analysis of focal therapy by Imperial Prostate and UK colleagues, reporting 1⃣0⃣-year outcomes after focal HIFU and cryotherapy in 3⃣4⃣7⃣7⃣ men 🔥🧊 Led by @AJWLight @LondonProstate1 @TaimurShah Read for free: europeanurology.com/article/…

The #RADAR Study is now open!🚨 High risk men aged 40-49 will have a #Prostagram scan, a Stockholm3 test, and a PSA test to find the best way to screen for #ProstateCancer. Check if you can sign up here: imperial.eu.qualtrics.com/jf… @NIHRresearch @ImperialSandC @ImperialNHS

Interesting TARGET international consensus guidance - we had uncertainty as to whether the pre-treatment MRI needs DCE (Eur Urol 2024; europeanurology.com/article/…) But PI-FAB, published by the same institution as PRIME, requires comparison with the pre-treatment DCE images 🤷
Great point-counterpoint at the podium today: @veerukasi arguing biparametric MRI should be the new standard of care for prostate cancer diagnosis vs @RaisBahrami making the case to keep contrast. PRIME (JAMA 2025) says bpMRI is noninferior to mpMRI for csPCa (29.0% vs 29.6%, 555 men, 22 centers). Shorter scan, no gadolinium. The counterargument: DCE still helps with equivocal PI-RADS 3 lesions and better defines lesion size, which matters for focal therapy planning. Where do you land? @FocalSociety @Endo_Society #ProstateCancer #bpMRI #PRIMEtrial #FTS2026 @JAMA_current

PSA to all my urologic cancer colleagues. Stop personally treating PCa. We should refer all prostate pts to @EvanThomas84. His outcomes are "perfect." NEVER has had a patient with urinary issues after prostate radiation. In fact they have better function.
Replying to @PGomellaMD
Do you know why? Because I care about my men’s post treatment QOL, and their urinary and sexual function is medically optimized before I start treatment. Everyone gets a PDE5 inhibitor, LUTS optimization, and either a cox inhibitor or dex through treatment. So yes, my men’s IPSS and SHIM typically go up after I meet them, and then stay that way through treatment. And I have NEVER left a man in pads, whether in my time in academia or since I opened a freestanding. Curious if you can say the same.

Radiation oncologists against any other treatment option: "We need data!". "Urologists are charlatans and bad surgeons!". Also their rebuttals include anecdotal comments about their overflowing clinics filled with endless focal therapy disasters.
If I had a nickel for every time a friend asked me about prostate focal therapy, I’d be rich. I’ve personally managed 20–30 intraprostatic failures after focal therapy. One patient ended up with a permanent Foley after salvage RT. Nearly all required medical castration, which many likely could have avoided with definitive therapy upfront. The downstream consequences are real. Our group ultimately decided not to offer focal therapy outside of very niche scenarios (for example select reirradiation not ideal for partial or full gland SBRT). If I were a patient, I might assume focal therapy is better than standard options too. The marketing is that good. #OncTwitter

We have just read a Letter to the Editor from @nickva1, @alison_tree, @piet_ost that carries an implication of dishonesty, and unfaith, deliberately distorting the facts in order to criticize and mislead readers to truly understand the article . “Oncological Outcomes Following Focal HIFU and Cryotherapy for Treatment of Nonmetastatic Prostate Cancer in the United Kingdom: An Updated Analysis of 3477 Patients from the Prospective HEAT and ICE Registries” We firmly oppose their behavior. Our forthcoming analysis will clarify the actual content and specifically identify how they have twisted the information. @LondonProstate1 ,@AJWLight @IP_London

Never and always are two words that should not be used in discussions with patients. I'll await your published 12-15 year oncologic and functional outcome from your personal prostate cancer panel showing 100%cancer control, 0%BCR and improved IPSS and SHIMs

@Archana_GK1 presented RAPID 5-year #ActiveSurveillance outcomes at @SGU2026 in St Gallen ➡️ ~50% treatment within 5 years, GG2>GG1 ➡️ Treatment triggers are heterogenous ➡️ Active surveillance needs improvement ➡️ IP9-ATLAS RCT recruiting (MRI-guided vs NICE SOC surveillance)

Presumably SBRT for localised favourable intermediate PCa will be rolled back following this RCT of 698 patients randomised in 136 international centres; designed and powered a priori as superiority of cancer control for SBRT over MH-IMRT, but wasn’t? jamanetwork.com/journals/jam…
🚨 5 fractions vs 20–28 fractions in intermediate-risk prostate cancer: NRG-GU005 Phase 3 RCT, 698 patients: SBRT 36.25 Gy/5 fx vs MH-IMRT 60 Gy/20 fx or 70 Gy/28 fx. 📊 Key results • 3-y DFS: 88.6% vs 92.1% with SBRT vs MH-IMRT • SBRT was not superior for DFS • Biochemical failure was higher with SBRT • Bowel QoL decline at 2 y: 34.9% vs 43.8%, favoring SBRT • Grade 3–4 GU toxicity: 0.6% vs 2.5%, favoring SBRT 💡 Why it matters SBRT offers major convenience and somewhat better QoL/toxicity, but this modest-dose 5-fraction regimen did not improve cancer control. ⚠️ The DFS difference was not statistically significant after covariate adjustment, but PSA failure was higher with SBRT. 🩺 Clinical verdict: PROMISING FOR CONVENIENCE, BUT DO NOT ASSUME EQUIVALENT DOSE INTENSITY @ASCO @oncoalert #ProstateCancer #RadiationOncology #SBRT
