馃У Presented at #ANES26: Among high-risk patients undergoing elective major abdominal surgery, individualized #CardiacIndex-guided hemodynamic therapy targeting each patient鈥檚 preoperative resting cardiac index did not reduce a composite of acute kidney injury, acute myocardial injury, severe infectious complications, nonfatal cardiac arrest, or death within 7 postoperative days vs routine hemodynamic therapy.
These findings do not support this strategy for routine intraoperative management in this surgical population.

馃搳 Research Summary: In high-risk elective major abdominal surgery, individualized #HemodynamicTherapy maintained intraoperative cardiac index closer to preoperative resting values but did not improve the primary postoperative outcome vs routine care.

馃挰 Viewpoint by Gayatri Devi, MD, MS: Blood-based biomarkers for #AlzheimerDisease may help diagnose symptomatic patients, but their role in asymptomatic adults remains uncertain.
In people without symptoms, positive test results may have limited predictive value for future cognitive decline and may introduce psychological, financial, insurance, and employment consequences without established preventive benefit.

Under the Increasing Organ Transplant Access model by the Centers for Medicare & Medicaid Services (CMS), most participating #KidneyTransplant centers publicly posted waitlist selection criteria, but about 1 in 3 did not.
Among centers that posted criteria, most materials exceeded recommended reading levels and were rarely available in non-English languages, indicating that policy compliance did not consistently translate into functional transparency for patients.

馃搳 JAMA Clinical Guidelines Synopsis: The 2025 @IDSAInfo guideline for adults with c omplicated #UrinaryTractInfection recommends selecting empirical antibiotic therapy based on severity of illness, risk factors for resistance, patient-specific considerations, and recent local antibiogram data.
For patients who are clinically improving, the guidance supports shorter treatment courses and transition from intravenous to oral antibiotics when an effective oral option is available.

Among individuals of African ancestry, a common #CD36 nonsense variant was associated with higher risk of dilated #cardiomyopathy in a dose-dependent pattern, with homozygous carriers having approximately 5-fold higher risk.
In 4 US cohorts, this variant accounted for more of the dilated cardiomyopathy burden than #TTN truncating variants, indicating that current genetic frameworks may miss a major contributor in this population.

What does it take to lead a successful clinical trial?
This week in Tanzania, Kirsten Bibbins-Domingo, PhD, MD, MAS, Editor in Chief of JAMA and the JAMA Network, and JAMA Associate Editor Michael Lipnick, MD, will teach The Fine Art of Clinical Trials, a long-running international course focused on the skills required to design, lead, and sustain impactful clinical research.
Held in Africa for the first time, the course aims to support researchers in developing expertise in trial leadership, strategy, governance, funding, and conducting studies in settings with limited resources while expanding regional and global research collaborations.

The Fine Art of Clinical Trials course is coordinated by
@anzicrc at Monash University in partnership with the African Acute Care Trials Network (AACUTNet), bringing together faculty from Africa, Australia, and the broader global clinical research community.
Drs Bibbins-Domingo and Lipnick are among the international faculty contributing to this effort to strengthen clinical trial leadership and expand opportunities for Africa-led participation in global research.
In this editorial, Roy H. Perlis, MD, MSc, Annette Flanagin, RN, MA, and Kirsten Bibbins-Domingo, PhD, MD, MAS discuss the launch of @JAMA_AI_, a new JAMA Network journal dedicated to publishing rigorous, clinically focused research spanning AI, digital health, and computational health.

#MedNews: Reducing lipoprotein(a) levels was widely assumed to be the next big breakthrough in cardiovascular disease, and European and US clinical guidelines already recommend measuring Lp(a) in all adults.
But after the Lp(a)-lowering agent pelacarsen failed to prevent major adverse cardiovascular events in a phase 3 trial, many are wondering what comes next.

Among patients with #SepticShock undergoing surgical source control, intraoperative dobutamine plus norepinephrine reduced new-onset or worsening #AcuteKidneyInjury within 7 days after surgery compared with norepinephrine alone. These findings support consideration of this intraoperative hemodynamic strategy for reducing postoperative kidney injury in this surgical population.

馃挕 JAMA Insights: In the US, #Autopsy remains important for #PublicHealth surveillance, cause-of-death determination, and identification of diagnostic errors, treatment complications, and emerging disease patterns.
Yet fewer than 8% of deaths undergo autopsy, and consented hospital autopsy rates have declined substantially over the past 60 years.

馃搫 JAMA Patient Page: An #Autopsy is a medical examination performed by pathologists that can help identify an individual鈥檚 cause of death.
This JAMA Patient Page explains the 2 main types of autopsy in the US, how often autopsies are performed, and how they can identify unsuspected diagnoses, complications of disease or treatment, and findings relevant to family members and #PublicHealth.

In mechanically ventilated adults with #AcuteHypoxemicRespiratoryFailure, an inhaled sedation strategy using isoflurane with intravenous adjuncts as needed did not reduce hospital mortality vs intravenous sedation. Ventilator-free days, intensive care unit鈥揻ree days, hospital-free days, and 3-month quality-of-life and disability scores were also similar between groups.
