Imagine reaching the point where apparently shoplifting from Greggs in broad daylight feels completely routine.
Take what you want. Walk out. Leave the staff to deal with it.
How did we get to the stage where this looks so normal?

This really worries me
A month ago in Wales I suffered a ruptured aneurysm in my abdomen. I lost over 2 units of blood
But the Welsh ambulance service refused to send an ambulance. I was still breathing so apparently didn't need one
I spent 7 hours lying on the ground in a car park. Every time I moved I threw up from the pain. The owners of the car park called 999 6x
One of the people there was a fireman. He couldn't believe that 999 treated each call as a separate incident and couldn't see the details or link to previous calls. He was frustrated because they could see I was seriously ill but you can't see internal bleeding and so there was no way to persuade 999 that it actually was an emergency
Eventually my husband arrived by taxi, journey of more than 3 hours from our home
He gave me my pain meds (the car park people were worried about liability and I was too ill to get them myself). This meant I was able to crawl into the car and he drove me to A&E
He got me into a wheelchair. We waited 75 minutes to see a doctor. I was shivering, heaped with blankets and threw up all over the floor
As soon as a doctor looked at me I was taken straight to resus. The next day I was transfered by blue light ambulance to another hospital, had a blood transfusion and spent 5 days on the high dependency unit
If my husband hadn't been able to come and look after me I have no idea how I would have survived. As it was I nearly didn't
I would not have been able to get myself to hospital nor would I have been able to log into some digital triage system
This scheme seems to assume if you're seriously ill you'll arrive by ambulance and if not you're well enough to navigate a digital portal
My experience suggests that's a dangerous assumption
A week later, back home in England I had another ruptured aneurysm. This time an ambulance came in 2 hours and again I was taken straight to resus
It wasn't the same because I had a recent diagnosis of a ruptured aneurysm so we could tell 999 I was almost certainly bleeding internally. But I was too ill to get myself down the stairs and out to the car. We still needed that ambulance and I still wouldn't have been able to fiddle around with an ipad
Proper triage REQUIRES an actual doctor to look at the patient. It takes a matter of minutes to differentiate between a life threatening emergency and not a life threatening emergency. That's not minutes to get a diagnosis but to know that the person is stable or not stable and if not that needs immediate attention
Seriously ill people can't do it themselves. It doesn't matter how smart or articulate they are normally. Or how tough. Expecting people to manage their own emergency care isn't what a modern health service should do

🧵Your Favourite Treatment Isn’t the Whole of Psychiatry 🚨1/16
This image shows the range of targets we deal with in psychiatry.
And it also shows why psychiatry becomes ‘dangerous’ when one target becomes the whole model.
There are broadly 3 layers of targets.
Let’s explore 👇

Additionally resident Drs are typically in the top of their school year with incredibly high STEM A level scores, they spend 5-6 years studying and finish with £100k debts. I took 19 years to get a consultant post, no one else trains for that long in the NHS - it’s a tough job!

Medical student experience of an encounter with a patient with delirium -
"Mrs. P was 83. She was in a bay of 4 patients on a medical ward. Two of them were noisy - one was calling out repeatedly, another had a loud TV.
Mrs. P was quiet. Eyes open, staring at the ceiling. Looked unhappy and puzzled. Not eating the lunch tray in front of her. Not pressing her call bell. Not moving much.
In a busy ward, quiet patients are 'settled' patients. Nobody seemed worried about Mrs. P.
I was taking histories as part of my clinical attachment. I sat down next to her and said hello and introduced myself. She looked at me briefly but didn't answer. She looked away. I asked her name. She did not answer. I asked her again and she gave her name. I asked where she was. She looked surprised by the question then said, 'at home'.
She had no idea she was in hospital.
I mentioned this to the ward doctor. He came and assessed her. She was profoundly inattentive, disoriented in time and place, and had been like this for at least two days based on the nursing observations - which had documented 'settled' and 'comfortable' because she wasn't causing any problems, though two entries also mentioned 'confused' ? baseline - though no additional mental status assessments had been done.
She had pneumonia and constipation that were already being treated, but nobody had formally recognised that her quietness was delirium. Hypoactive delirium. The subtype where patients don't shout, don't climb out of bed, don't pull out their lines. They just... go quiet.
And going quiet, in a hospital, can make you less visible.
Hypoactive delirium is more common than the hyperactive form, and it carries a worse prognosis. Patients with hypoactive delirium have higher mortality rates, and longer hospital stays. Yet it's detected even less often, because it doesn't trigger alarms. It doesn't create workload. It doesn't disrupt the ward.
The noisiest patients get the most attention but the quietest patients may be sicker.
Since that day, I make a point of doing a delirium assessment on the quiet ones."

NEW: I FOIed HMRC again regarding McCloud rectification, here are some insights into the data they sent me yesterday 18/02/26 in their 8th extract (table below)
- Only 23,380 submissions across whole of public sector, lots more RPSS sent out, fewer than 1 in 7 NHS issued statements have been processed!
- NHS submissions are the vast majority at 16,180 (67.7%) no surprise there doctors are the biggest group *by far* paying annual allowances charges but that's a tiny fraction of all the RPSS sent out in the NHS, so lots of people *are* ignoring these ?!?!
- DONT IGNORE these - most people getting refunds/compensation with avg over £14k - if you don't apply you aint getting that cash back
- 55% user submitted, 45% using an agent - remember via the NHS cost claimback its fully compensable to use an expert accountant - its complex & time consuming with lots of pitfalls - you don't need to do this yourself! That message is starting to sink in which a greater proportion now using an agent
- It's complex! The risk of errors and risk of not claiming the the right amounts are significant, so I and others would strongly advise using a specialist medical accountant (i.e. AISMA) - that's what they are professionally accountable to do! All our claims are reviewed by TWO accountants
- HMRC have clearly been working overtime since last FOI with outstanding processing down 90% so HMRC processing should now be measurable in days/weeks, not months!
Remember - you will qualify for FREE expert accountancy advice if you

✅Pension growth ↑ in any yr 15/16-21/22 post-roll*
✅Pension growth ↑ £40k in 22/23
* difficult to determine as NHS pensions decided to no longer include old growth - NOT HELPFUL

If in trying to prevent young suicide we start not with a social media ban but earlier, with the multiple factors that lead to tragedy, we would target child abuse, isolation, bullying, exam pressure, autism, being in care, self-harm &, also, online risk.
