A Churchill Fellowship report on ‘Preventing Doctors From Dying By Suicide’, written by our Trustee Dr Ananta Dave, was officially launched at the Royal College of Psychiatrists.
Dr Ananta Dave, who is the Presidential lead for retention and wellbeing at the Royal College of Psychiatrists, wrote the paper to provide practical guidance for organisations to understand risk factors involving the suicide of doctors. She makes a number of recommendations for the next steps to take to reduce suicides in healthcare workers.
A special reception was held yesterday to mark the launch and Doctors in Distress Chief Executive Ann Paul was in attendance.

I'm preparing to record a film on "belonging at work" for an MBA module. Belonging creates a profound sense of connection & validation at work. When we feel we belong, we thrive & so do the people around us.
We feel a sense of belonging when we:
1) are seen for our unique contributions;
2) bond with the people we work with
3) are supported in our daily work & career development;
4) `feel proud of our organisation’s values and purpose.
We need to build belonging not just within our home work teams but for project teams & multidisciplinary/ cross-cutting teams. It's one of the most powerful ways to boost the conditions for better outcomes.

This will be a big big problem with ICD-11 as it gets rolled out & every clinician is encouraged to apply 'personality difficulty' labels based on generally uninformed and brief impressions. These will go on to cause a lot of harm. We already know this & yet it seems inevitable.

One of the most common tests I use in clinical practice.
Examination of fronto-striatal circuits.
In Psychiatry we have clinical examination markers that provide us with diagnostic, treatment and prognostic information for a formulation.
Here are some of the aspects about the Luria 3- step test
1. Set -Shifting - A frontal lobe function
2. Use as activation test to examine for Tardive dyskinesia & set -shifting at the same time
3. Intention tremors / ‘smoothness’ / perseveration
4. Slowness
5. Asymmetry
6. Divergence between ‘diagnosis’ and test performance e.g Diagnosis of Depression ( non- melancholic) and significant abnormality in performance 👉high probability of organic involvement
7. Treatment marker ( before and after) set shifting improves with appropriate treatment
8. Psychosocial aspects - if someone is struggling with 3-step set shifting - imagine having to fill in forms for disability or to go shopping, make lists etc.
9. Antipsychotic SE - Degree of frontal blockade - very useful in dose titration
10. Ruling out neurodegenerative conditions ( this needs a more comprehensive evaluation)
What else?
