8) 🏥 costs for ATTR-CM: similar to non-amyloid HF, but >> non-HF controls. 📍All-cause inpatient: $32,960 vs $32,076 (P=0.214) 📍Combined costs: $35,983 vs $34,980 (P=0.382) 📍vs non-HF: $32,960 vs $29,561 & $35,983 vs $32,284 (both P<0.001) ATTR econ burden remains huge.

Genotype and arrhythmic risk in #DCM : International Sarcomeric Human Cardiomyopathy Registry (SHaRe) 📍2,660 adults with DCM: 16% carried pathogenic/likely pathogenic variants in high-arrhythmogenic-risk genes; 17% in low-risk genes; 67% were gene-elusive. 📍3.25× higher risk of major VAs in patients with high-risk genetic variants vs. gene-elusive patients. 📍 Beyond arrhythmias: High-risk variants were associated with ⬆️ risks of #AFib (HR 2.42) and #HeartFailure (HR 2.46). 📍Preserved #LVEF doesn’t eliminate risk: Even with LVEF ≥50%, high-risk variant carriers had a 2.36× higher risk of major #VAs academic.oup.com/eurheartj/a… @ESC_Journals @ehj_ed

Its not about more or less fluids. Its about giving patients the right hemodynamic support based on their physiology! Some patients in early septic shock are: 1. Fluid responsive with low SV --> fluids 2. Fluid unresponsive with low SV --> pressors, inotropes 3. Fluid responsive with high SV --> pressors 4. Fluid unresponsive with high SV --> pressors 5. Congested --> inotropes + diuresis 6. Impaired microcirculation with hemodynamic coherence --> fix macrohemodynamics 7. Impaired microcirculation with hemodynamic incoherence --> MAP target, avoid edema, other? (anticoagulation) We need to stop treating "septic shock" as hemodynamic paradigm.
Among adults with septic shock, the use of restricted fluids and early vasopressors did not result in more days alive and out of the hospital through day 90 than the use of liberal fluids and later vasopressors. Full ARISE FLUIDS trial results and Research Summary: nej.md/3PVdwIp

In #HFpEF, prominent #PAWP V waves may identify a high-risk LA myopathy phenotype. Among 1,069 patients undergoing invasive exercise testing, 21% had prominent V waves and showed: 📍 Larger #LAVI: 42.2 vs 32.5 mL/m²
📍 Lower LA reservoir strain: 16.4% vs 23.2%
📍 AF: 61.3% vs 22.9%
📍 PA systolic pressure: 54 vs 38 mmHg
📍 Lower PA compliance: 3.03 vs 4.90 mL/mmHg V-wave correction reduced apparent combined pre/post-capillary PH from 60.2% → 29.3%. Prominent V waves were associated with worse RV function/exercise reserve and 2× higher risk of HF 🏥 or ☠️ (adjusted HR 2.09). ahajournals.org/doi/10.1161/… @CircAHA

🧬 From phenotype to precision medicine in #Cardiomyopathy An insightful #EJHF review explores how genetics, advanced imaging & deep phenotyping can improve the management of DCM & NDLVC: 📍DCM & NDLVC represent a disease continuum, sharing genetic backgrounds but with potentially different clinical trajectories. 📍Phenotype is the starting point, not the final diagnosis. Integrate clinical assessment, ECG, echo, CMR & genetics. 📍Genotype matters. Different variants are associated with distinct clinical trajectories, for example; - LMNA: Conduction disease & arrhythmias - FLNCtv: High arrhythmic burden, fibrosis & SCD risk - DSP: Arrhythmias, fibrosis & myocarditis-like episodes 📍Arrhythmic risk goes beyond LVEF. Consider genotype, arrhythmia burden & CMR fibrosis/LGE when assessing risk and ICD decisions. 📍Consider genetic testing in DCM/NDLVC. Results may guide family screening, risk stratification & management 📍Precision medicine is evolving. Gene-directed therapies are emerging, while GDMT remains the cornerstone of HF management.
