Critical illness triggers severe systemic inflammation, driving dysregulation of the HPA axis, altered cortisol clearance, and peripheral glucocorticoid receptor resistance.
The result? An inadequate intracellular anti-inflammatory response despite seemingly "normal" total circulating cortisol levels. This underpins hydrocortisone use in refractory vasopressor-dependent septic shock.
